REPLACEMENT OF MISSING TEETH
Missing teeth directly influences the quality of life. When left unreplaced over a period of time, missing teeth may present serious problems like any other dental disease. So it is important to replace the missing teeth at the earliest. Immediately after tooth loss functions such as eating and speech may be handicapped which will worsen over time. A gap is created in the jaw when a tooth is removed. Eventually the other teeth slowly shifts into this gap resulting in crooked arrangement of teeth. Each tooth is in position by the support of the adjacent teeth. A missing tooth will alter this system and will cause overall weakening of teeth which in turn will result in more tooth loss.
Tooth replacement can be done by
Dentures
Bridges
Dental Implants
DENTURES
Dentures are removable appliance that replaces the missing teeth. It serves the aesthetic and functional requirements but carries the discomfort of removing every night. This is not a permanent solution as the dentures are generally made of Polymethlmethacrylate which is not biologically stable. So the dentures has to be periodically replaced to prevent any adverse effect of PMMA and to preserve the underlying bone.
Newer advancements like BioFunctional Prosthetic System uses denture base materials that are highly compatible with the sensitive oral mucosa. The materials are available in various shades to maintain gum aesthetics and the artificial teeth used in this system mimic the natural teeth. The recordings of the jaw are made with special devices called gnathometer and is transferred to the semi adjustable particular, the device that closely mimics the human jaw. The teeth are then arranged on this so that the replaced teeth are located and functions as close to the previously existing natural teeth.
BRIDGES
Bridges are fixed dental prosthesis that are attached to the adjacent tooth mechanically by cement. The advantage of a bridge is it is fixed.Hence there is increased comfort. In case of long span edentulous space when there is not enough tooth to support the bridge, Implants could be placed to support the bride. The bridge could be either screwed or cemented onto the implant.
The bridges can be made of metals like cobalt-chromium,Gold and Titanium, Porcelain fused to metal, aesthetic all ceramics and newer Zirconia. The CAD-CAM system has changed the old system of casting and now more precise adaptation and aesthetics of the bridge are achieved by laser sintering and milling.
DENTAL IMPLANTS
Dental Implants are the most recent method of tooth replacement. It is superior to other replacement methods owing to its anchorage to the bone and increased sensation of the chewing forces to the brain. The other replacement systems like removable denture and tooth supported fixed partial dentures are just mechanically retained and do not send the sensory impulses to the brain.
Dental Implants are titanium screws that are placed in the bone. The implant replaces the root portion of the tooth. The crown portion of the tooth that is outside the gum is then placed with either screw or cement retention to the underlying implant.
The dental implants are placed under local anaesthesia by a minor surgical drilling of the bone. The implants can be used to replace single tooth or all the teeth that are missing. The procedure is done as an outpatient treatment where the patient can walk out with a tooth
For patients in whom the adequate bone is not available to place implants, still the replacement could be done with implants by additional procedures like bone grafting and sinus lift. Best Dental Implants are the best solution for aesthetic and functional replacement of tooth.
